You are required to report your Port of Seattle workplace incident within 24 hours. 

Contractor injuries, if you are not employed by the Port of Seattle or if this is regarding any Risk/Liability incident they are not reported on this form.

Port of Seattle Employee Injury (No medical treatment): Employees are required to submit an incident report for any work related injury, illness or exposure.  

If you are not seeking medical treatment at this time, please select the "Report Employee Injury-Not Seeking Medical Treatment" option show below to report your injury.  

You have one year to file a claim should you decide to seek medical treatment.  

Port of Seattle Employee Injury (Seeking medical treatment):   

Selecting this option will automatically file a workers' compensation claim. Once a claim is filed, you will receive an email with your claim form attached.    

Once you seek medical treatment, you will not be permitted to return to work without a work release.
